Yeah, that would be OH, right here in Cleveland. I think the last time it happened was sometime in the early 70s, but it happened a bunch of times. Stupid steel mills dumped all sorts of crud in the river, and sometimes it would just spontaneously combust.
Made for easy suicides, though. No one would try to save you if you jumped, and everyone knew the intent if they saw someone go in.

The river's much cleaner now. There are fish in it again, and it's particularly popular with trout fishers. I have a bunch of friends who are members of the Western Reserve Rowing Association (
http://www.wrra.cc/news.php), and they row on the Cuyahoga. They say it's not much fun to capsize, but it's mostly because the water's cold and really deep, not because of pollution.
Still, we get our water from Lake Erie, not the river.
